What to bring to your first appointment.

• All related medical records, including diagnostic reports (X-rays, MRI, etc.)
• A valid insurance card and photo id.
• A list of current prescriptions and/or over-the-counter
  medications you are taking.
• If appropriate and required, bring the referral from your referring physician.
